nfs: add missing selections of CONFIG_CRC32

nfs.ko, nfsd.ko, and lockd.ko all use crc32_le(), which is available
only when CONFIG_CRC32 is enabled. But the only NFS kconfig option that
selected CONFIG_CRC32 was CONFIG_NFS_DEBUG, which is client-specific and
did not actually guard the use of crc32_le() even on the client.

The code worked around this bug by only actually calling crc32_le() when
CONFIG_CRC32 is built-in, instead hard-coding '0' in other cases. This
avoided randconfig build errors, and in real kernels the fallback code
was unlikely to be reached since CONFIG_CRC32 is 'default y'. But, this
really needs to just be done properly, especially now that I'm planning
to update CONFIG_CRC32 to not be 'default y'.

Therefore, make CONFIG_NFS_FS, CONFIG_NFSD, and CONFIG_LOCKD select
CONFIG_CRC32. Then remove the fallback code that becomes unnecessary,
as well as the selection of CONFIG_CRC32 from CONFIG_NFS_DEBUG.

nfsd: new Kconfig option for legacy client tracking

We've had a number of attempts at different NFSv4 client tracking
methods over the years, but now nfsdcld has emerged as the clear winner
since the others (recoverydir and the usermodehelper upcall) are
problematic.

As a case in point, the recoverydir backend uses MD5 hashes to encode
long form clientid strings, which means that nfsd repeatedly gets dinged
on FIPS audits, since MD5 isn't considered secure. Its use of MD5 is not
cryptographically significant, so there is no danger there, but allowing
us to compile that out allows us to sidestep the issue entirely.

As a prelude to eventually removing support for these client tracking
methods, add a new Kconfig option that enables them. Mark it deprecated
and make it default to N.

NFS & NFSD: Update GSS dependencies

> CONFIG_CRYPTO_CBC=m
> CONFIG_CRYPTO_CTS=m
> CONFIG_CRYPTO_ECB=m
> CONFIG_CRYPTO_HMAC=m
> CONFIG_CRYPTO_MD5=m
> CONFIG_CRYPTO_SHA1=m

This behavior is triggered by the "default y" in the definition of
RPCSEC_GSS.

The "default y" was added in 2010 by commit df486a25900f ("NFS: Fix
the selection of security flavours in Kconfig"). However,
svc_gss_principal was removed in 2012 by commit 03a4e1f6ddf2
("nfsd4: move principal name into svc_cred"), so the 2010 fix is
no longer necessary. We can safely change the NFS_V4 and NFSD_V4
dependencies back to RPCSEC_GSS_KRB5 to get the nicer v6.2
behavior back.

Selecting KRB5 symbolically represents the true requirement here:
that all spec-compliant NFSv4 implementations must have Kerberos
available to use.

nfsd: allow disabling NFSv2 at compile time

rpc.nfsd stopped supporting NFSv2 a year ago. Take the next logical
step toward deprecating it and allow NFSv2 support to be compiled out.

Add a new CONFIG_NFSD_V2 option that can be turned off and rework the
CONFIG_NFSD_V?_ACL option dependencies. Add a description that
discourages enabling it.

Also, change the description of CONFIG_NFSD to state that the always-on
version is now 3 instead of 2.

Finally, add an #ifdef around "case 2:" in __write_versions. When NFSv2
is disabled at compile time, this should make the kernel ignore attempts
to disable it at runtime, but still error out when trying to enable it.

NFSD: Remove CONFIG_NFSD_V3

Eventually support for NFSv2 in the Linux NFS server is to be
deprecated and then removed.

However, NFSv2 is the "always supported" version that is available
as soon as CONFIG_NFSD is set. Before NFSv2 support can be removed,
we need to choose a different "always supported" version.

This patch removes CONFIG_NFSD_V3 so that NFSv3 is always supported,
as NFSv2 is today. When NFSv2 support is removed, NFSv3 will become
the only "always supported" NFS version.

The defconfigs still need to be updated to remove CONFIG_NFSD_V3=y.

nfsd: depend on CRYPTO_MD5 for legacy client tracking

The legacy client tracking infrastructure of nfsd makes use of MD5 to
derive a client's recovery directory name. As the nfsd module doesn't
declare any dependency on CRYPTO_MD5, though, it may fail to allocate
the hash if the kernel was compiled without it. As a result, generation
of client recovery directories will fail with the following error:

NFSD: unable to generate recoverydir name

The explicit dependency on CRYPTO_MD5 was removed as redundant back in
6aaa67b5f3b9 (NFSD: Remove redundant "select" clauses in fs/Kconfig

This broke when RPCSEC_GSS_KRB5 was made optional for NFSv4 in commit

Deprecate nfsd fault injection

This is only useful for client testing. I haven't really maintained it,
and reference counting and locking are wrong at this point. You can get
some of the same functionality now from nfsd/clients/.

It was a good idea but I think its time has passed.

In the unlikely event of users, hopefully the BROKEN dependency will
prompt them to speak up. Otherwise I expect to remove it soon.

nfsd: add a new struct file caching facility to nfsd

Currently, NFSv2/3 reads and writes have to open a file, do the read or
write and then close it again for each RPC. This is highly inefficient,
especially when the underlying filesystem has a relatively slow open
routine.

This patch adds a new open file cache to knfsd. Rather than doing an
open for each RPC, the read/write handlers can call into this cache to
see if there is one already there for the correct filehandle and
NFS_MAY_READ/WRITE flags.

If there isn't an entry, then we create a new one and attempt to
perform the open. If there is, then we wait until the entry is fully
instantiated and return it if it is at the end of the wait. If it's
not, then we attempt to take over construction.

Since the main goal is to speed up NFSv2/3 I/O, we don't want to
close these files on last put of these objects. We need to keep them
around for a little while since we never know when the next READ/WRITE
will come in.

Cache entries have a hardcoded 1s timeout, and we have a recurring
workqueue job that walks the cache and purges any entries that have
expired.

This should shrink the kernel size for embedded device that only use,
say eMMC a bit.
