To prepare for completely separating the VGA console screen_info from the one used in EFI/sysfb, rename the vgacon instances and make them local as much as possible.
ia64 and arm both have confurations with vgacon and efi, but the contents never overlaps because ia64 has no EFI framebuffer, and arm only has vga console on legacy platforms without EFI. Renaming these is required before the EFI screen_info can be moved into drivers/firmware.
The ia64 vga console is actually registered in two places from setup_arch(), but one of them is wrong, so drop the one in pcdp.c and fix the one in setup.c to use the correct conditional.
x86 has to keep them together, as the boot protocol is used to switch between VGA text console and framebuffer through the screen_info data.

On non-x86 architectures, the screen_info variable is generally only used for the VGA console where supported, and in some cases the EFI framebuffer or vga16fb.
Now that we have a definite list of which architectures actually use it for what, use consistent #ifdef checks so the global variable is only defined when it is actually used on those architectures.
Loongarch and riscv have no support for vgacon or vga16fb, but they support EFI firmware, so only that needs to be checked, and the initialization can be removed because that is handled by EFI. IA64 has both vgacon and EFI, though EFI apparently never uses a framebuffer here.

There are several purposes of doing this: - dropping dependency in bug.h - dropping a loop by moving out panic_notifier.h - unload kernel.h from something which has its own domain
At the same time convert users tree-wide to use new headers, although for the time being include new header back to kernel.h to avoid twisted indirected includes for existing users.

SPARSEMEM memory model was supposed to entirely replace DISCONTIGMEM a (long) while ago. The last architectures that used DISCONTIGMEM were updated to use other memory models in v5.11 and it is about the time to entirely remove DISCONTIGMEM from the kernel.
This set removes DISCONTIGMEM from alpha, arc and m68k, simplifies memory model selection in mm/Kconfig and replaces usage of redundant CONFIG_NEED_MULTIPLE_NODES and CONFIG_FLAT_NODE_MEM_MAP with CONFIG_NUMA and CONFIG_FLATMEM respectively.
I've also removed NUMA support on alpha that was BROKEN for more than 15 years.
There were also minor updates all over arch/ to remove mentions of DISCONTIGMEM in comments and #ifdefs.
This patch (of 9):
NUMA is marked broken on alpha for more than 15 years and DISCONTIGMEM was replaced with SPARSEMEM in v5.11.
Remove both NUMA and DISCONTIGMEM support from alpha.

It's been a while since DISCONTIGMEM is generally considered deprecated, but it is still used by four architectures. This set replaces DISCONTIGMEM with a different way to handle holes in the memory map and marks DISCONTIGMEM configuration as BROKEN in Kconfigs of these architectures with the intention to completely remove it in several releases.
While for 64-bit alpha and ia64 the switch to SPARSEMEM is quite obvious and was a matter of moving some bits around, for smaller 32-bit arc and m68k SPARSEMEM is not necessarily the best thing to do.
On 32-bit machines SPARSEMEM would require large sections to make section index fit in the page flags, but larger sections mean that more memory is wasted for unused memory map.
Besides, pfn_to_page() and page_to_pfn() become less efficient, at least on arc.
So I've decided to generalize arm's approach for freeing of unused parts of the memory map with FLATMEM and enable it for both arc and m68k. The details are in the description of patches 10 (arc) and 13 (m68k).
This patch (of 13):
Enable SPARSEMEM support on alpha and deprecate DISCONTIGMEM.
The required changes are mostly around moving duplicated definitions of page access and address conversion macros to a common place and making sure they are available for all memory models.
The DISCONTINGMEM support is marked as BROKEN an will be removed in a couple of releases.

Patch series "mm: consolidate definitions of page table accessors", v2.
The low level page table accessors (pXY_index(), pXY_offset()) are duplicated across all architectures and sometimes more than once. For instance, we have 31 definition of pgd_offset() for 25 supported architectures.
Most of these definitions are actually identical and typically it boils down to, e.g.

static inline pmd_t *pmd_offset(pud_t *pud, unsigned long address) { return (pmd_t *)pud_page_vaddr(*pud) + pmd_index(address); }
These definitions can be shared among 90% of the arches provided XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined.
For architectures that really need a custom version there is always possibility to override the generic version with the usual ifdefs magic.
These patches introduce include/linux/pgtable.h that replaces include/asm-generic/pgtable.h and add the definitions of the page table accessors to the new header.
This patch (of 12):
The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the functions involving page table manipulations, e.g. pte_alloc() and pmd_alloc(). So, there is no point to explicitly include <asm/pgtable.h> in the files that include <linux/mm.h>.
The include statements in such cases are remove with a simple loop:

Since __sysrq_get_key_op() is no longer used externally, let's make it a static function.
This patch will allow us to limit access to each and every sysrq op and constify the sysrq handling.

The conversion of alpha to memblock as the early memory manager caused boot to hang as described at [1].
The issue is caused because for CONFIG_DISCTONTIGMEM=y case, memblock_add() is called using memory start PFN that had been rounded down to the nearest 8Mb and it caused memblock to see more memory that is actually present in the system.
Besides, memblock allocates memory from high addresses while bootmem was using low memory, which broke the assumption that early allocations are always accessible by the hardware.
This patch ensures that memblock_add() is using the correct PFN for the memory start and forces memblock to use bottom-up allocations.
