[debug-info] Add support for llvm.dbg.addr in DIBuilder.
I based this off of the API already create for llvm.dbg.value since both intrinsics have the same arguments at the API level.
I added some tests exercising the API a little as well as an additional small test that shows how one can use llvm.dbg.addr to limit the PC range where an address value is available in the debugger. This is done by calling llvm.dbg.value with undef and the same metadata info as one used to create the llvm.dbg.addr.

[DebugInfo] Check DIEnumerator bit width when comparing for equality
As mentioned in D106585, this causes non-determinism, which can also be shown by this test case being flaky without this patch.
We were using the APSInt's bit width for hashing, but not for checking for equality. APInt::isSameValue() does not check bit width.

[DebugInfo] Handle multiple variable location operands in IR
This patch updates the various IR passes to correctly handle dbg.values with a DIArgList location. This patch does not actually allow DIArgLists to be produced by salvageDebugInfo, and it does not affect any pass after codegen-prepare. Other than that, it should cover every IR pass.
Most of the changes simply extend code that operated on a single debug value to operate on the list of debug values in the style of any_of, all_of, for_each, etc. Instances of setOperand(0, ...) have been replaced with with replaceVariableLocationOp, which takes the value that is being replaced as an additional argument. In places where this value isn't readily available, we have to track the old value through to the point where it gets replaced.

[Instruction] Set metadata uses to undef on deletion
Summary: Replace any extant metadata uses of a dying instruction with undef to preserve debug info accuracy. Some alternatives include:
- Treat Instruction like any other Value, and point its extant metadata uses to an empty ValueAsMetadata node. This makes extant dbg.value uses trivially dead (i.e. fair game for deletion in many passes), leading to stale dbg.values being in effect for too long.
- Call salvageDebugInfoOrMarkUndef. Not needed to make instruction removal correct. OTOH results in wasted work in some common cases (e.g. when all instructions in a BasicBlock are deleted).
This came up while discussing some basic cases in https://reviews.llvm.org/D80052.

[DebugInfo] Update loop metadata in stripNonLineTableDebugInfo
Summary: Have stripNonLineTableDebugInfo() attach updated !llvm.loop metadata to an instruction (instead of updating and then discarding the metadata).
This fixes "!dbg attachment points at wrong subprogram for function" errors seen while archiving an iOS app.
It would be nice -- as a follow-up -- to catch this issue earlier, perhaps by modifying the verifier to constrain where DILocations are allowed. Any alternative suggestions appreciated.

Update the file headers across all of the LLVM projects in the monorepo to reflect the new license.
We understand that people may be surprised that we're moving the header entirely to discuss the new license. We checked this carefully with the Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM project under our new license, so you will see that the license headers include that license only. Some of our contributors have contributed code under our old license, and accordingly, we have retained a copy of our old license notice in the top-level files in each project and repository.
