[lldb/Symbol] Fix column breakpoint `move_to_nearest_code` match

This patch fixes the column symbol resolution when creating a breakpoint
with the `move_to_nearest_code` flag set.

In order to achieve this, the patch adds column information handling in
the `LineTable`'s `LineEntry` finder. After experimenting a little, it
turns out the most natural approach in case of an inaccurate column match,
is to move backward and match the previous `LineEntry` rather than going
forward like we do with simple line breakpoints.

The patch also reflows the function to reduce code duplication.

Finally, it updates the `BreakpointResolver` heuristic to align it with
the `LineTable` method.

[lldb] Refactor argument group by SourceLocationSpec (NFCI)

This patch refactors a good part of the code base turning the usual
FileSpec, Line, Column, CheckInlines, ExactMatch arguments into a
SourceLocationSpec object.

This change is required for a following patch that will add handling of the
column line information when doing symbol resolution.

[lldb][NFC] Fix all formatting errors in .cpp file headers

Summary:
A *.cpp file header in LLDB (and in LLDB) should like this:
```
//===-- TestUtilities.cpp -------------------------------------------------===//
```
However in LLDB most of our source files have arbitrary changes to this format and
these changes are spreading through LLDB as folks usually just use the existing
source files as templates for their new files (most notably the unnecessary
editor language indicator `-*- C++ -*-` is spreading and in every review
someone is pointing out that this is wrong, resulting in people pointing out that this
is done in the same way in other files).

This patch removes most of these inconsistencies including the editor language indicators,
all the different missing/additional '-' characters, files that center the file name, missing
trailing `===//` (mostly caused by clang-format breaking the line).

Fix line table resolution near the end of a section

Summary:
lld r367537 changed the way the linker organizes sections and segments.
This exposed an lldb bug and caused some tests to fail.

In all of the failing tests the root cause was the same -- when we were
trying to resolve the last address in the line_table section, we failed
because it pointed past the end of the section.

This patch changes the line table address resolution code to back up the
address by one for end-of-sequence entries. This ensures the address
still points inside a section/module even if the line table sequence
ends at the very end of a section.

It also reverts the linker flags which were added to the failing tests
to restore previous behavior.

[NFC] Remove ASCII lines from comments

A lot of comments in LLDB are surrounded by an ASCII line to delimit the
begging and end of the comment.

Its use is not really consistent across the code base, sometimes the
lines are longer, sometimes they are shorter and sometimes they are
omitted. Furthermore, it looks kind of weird with the 80 column limit,
where the comment actually extends past the line, but not by much.
Furthermore, when /// is used for Doxygen comments, it looks
particularly odd. And when // is used, it incorrectly gives the
impression that it's actually a Doxygen comment.

I assume these lines were added to improve distinguishing between
comments and code. However, given that todays editors and IDEs do a
great job at highlighting comments, I think it's worth to drop this for
the sake of consistency. The alternative is fixing all the
inconsistencies, which would create a lot more churn.

Replace 'ap' with 'up' suffix in variable names. (NFC)

The `ap` suffix is a remnant of lldb's former use of auto pointers,
before they got deprecated. Although all their uses were replaced by
unique pointers, some variables still carried the suffix.

In r353795 I removed another auto_ptr remnant, namely redundant calls to
::get for unique_pointers. Jim justly noted that this is a good
opportunity to clean up the variable names as well.

I went over all the changes to ensure my find-and-replace didn't have
any undesired side-effects. I hope I didn't miss any, but if you end up
at this commit doing a git blame on a weirdly named variable, please
know that the change was unintentional.

Update the file headers across all of the LLVM projects in the monorepo
to reflect the new license.

We understand that people may be surprised that we're moving the header
entirely to discuss the new license. We checked this carefully with the
Foundation's lawyer and we believe this is the correct approach.

Essentially, all code in the project is now made available by the LLVM
project under our new license, so you will see that the license headers
include that license only. Some of our contributors have contributed
code under our old license, and accordingly, we have retained a copy of
our old license notice in the top-level files in each project and
repository.

Move classes from Core -> Utility.

This moves the following classes from Core -> Utility.

ConstString
Error
RegularExpression
Stream
StreamString

The goal here is to get lldbUtility into a state where it has
no dependendencies except on itself and LLVM, so it can be the
starting point at which to start untangling LLDB's dependencies.
These are all low level and very widely used classes, and
previously lldbUtility had dependencies up to lldbCore in order
to use these classes. So moving then down to lldbUtility makes
sense from both the short term and long term perspective in
solving this problem.

*** This commit represents a complete reformatting of the LLDB source code
*** to conform to clang-format’s LLVM style. This kind of mass change has
*** two obvious implications:

Firstly, merging this particular commit into a downstream fork may be a huge
effort. Alternatively, it may be worth merging all changes up to this commit,
performing the same reformatting operation locally, and then discarding the
merge for this particular commit. The commands used to accomplish this
reformatting were as follows (with current working directory as the root of
the repository):

find . \( -iname "*.c" -or -iname "*.cpp" -or -iname "*.h" -or -iname "*.mm" \) -exec clang-format -i {} +
find . -iname "*.py" -exec autopep8 --in-place --aggressive --aggressive {} + ;

The version of clang-format used was 3.9.0, and autopep8 was 1.2.4.

Secondly, “blame” style tools will generally point to this commit instead of
a meaningful prior commit. There are alternatives available that will attempt
to look through this change and find the appropriate prior commit. YMMV.

Keep original source path and mapped path in LineEntry

Summary:
The "file" variable in a LineEntry was mapped using target.source-map, except when stepping through inlined code. This patch adds a new variable to LineEntry, "original_file", that contains the original file from the debug info. "file" will continue to (possibly) be mapped.

Some code has been changed to use "original_file". This is code dealing with symbols. Code dealing with source files will still use "file". Reviewers, please confirm that these particular changes are correct.

Fix prologue end handling when code compiled by gcc

GCC don't use the is_prologue_end flag to mark the first instruction
after the prologue. Instead of it it is issuing a line table entry for
the first instruction of the prologue and one for the first instruction
after the prologue. If the size of the prologue is 0 instruction then
the 2 line entry will have the same file address.

We remove these duplicates entries as they are violating the dwarf spec
and can cause confusion in the debugger. To prevent the lost of
information about the end of prologue we should set the prologue end
flag for the line entries what are representing more then 1 entry.
