[lldb] Make logging machinery type-safe
This patch makes use of c++ type checking and scoped enums to make logging statements shorter and harder to misuse.
Defines like LIBLLDB_LOG_PROCESS are replaces with LLDBLog::Process. Because it now carries type information we do not need to worry about matching a specific enum value with the right getter function -- the compiler will now do that for us.
The main entry point for the logging machinery becomes the GetLog (template) function, which will obtain the correct Log object based on the enum type. It achieves this through another template function (LogChannelFor<T>), which must be specialized for each type, and should return the appropriate channel object.
This patch also removes the ability to log a message if multiple categories are enabled simultaneously as it was unused and confusing.
This patch does not actually remove any of the existing interfaces. The defines and log retrieval functions are left around as wrappers around the new interfaces. They will be removed in follow-up patch.

[lldb][NFC] Fix all formatting errors in .cpp file headers
Summary: A *.cpp file header in LLDB (and in LLDB) should like this: ``` //===-- TestUtilities.cpp -------------------------------------------------===// ``` However in LLDB most of our source files have arbitrary changes to this format and these changes are spreading through LLDB as folks usually just use the existing source files as templates for their new files (most notably the unnecessary editor language indicator `-*- C++ -*-` is spreading and in every review someone is pointing out that this is wrong, resulting in people pointing out that this is done in the same way in other files).
This patch removes most of these inconsistencies including the editor language indicators, all the different missing/additional '-' characters, files that center the file name, missing trailing `===//` (mostly caused by clang-format breaking the line).

Update the file headers across all of the LLVM projects in the monorepo to reflect the new license.
We understand that people may be surprised that we're moving the header entirely to discuss the new license. We checked this carefully with the Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM project under our new license, so you will see that the license headers include that license only. Some of our contributors have contributed code under our old license, and accordingly, we have retained a copy of our old license notice in the top-level files in each project and repository.

Remove lldb streams from the Log class completely
Summary: previously we switched to llvm streams for log output, this completes the switch for the error streams.
I also clean up the includes and remove the unused argument from DisableAllLogChannels().
This required adding a bit of boiler plate to convert the output in the command interpreter, but that should go away when we switch command results to use llvm streams as well.

Refactor log channel registration mechanism
Summary: We currently have two log channel registration mechanisms. One uses a set of function pointers and the other one is based on the PluginManager.
The PluginManager dependency is unfortunate, as logging is also used in lldb-server, and the PluginManager pulls in a lot of classes which are not used in lldb-server.
Both approach have the problem that they leave too much to do for the user, and so the individual log channels end up reimplementing command line argument parsing, category listing, etc.
Here, I replace the PluginManager-based approach with a one. The new API is more declarative, so the user only needs to specify the list of list of channels, their descriptions, etc., and all the common tasks like enabling/disabling categories are hadled by common code. I migrate the LogChannelDWARF (only user of the PluginManager method) to the new API.
In the follow-up commits I'll replace the other channels with something similar.

Convert Log class to llvm streams
Summary: This converts LLDB's logging to use llvm streams instead of lldb_private::Stream and friends. The changes are mostly straight-forward and amount to s/lldb_private::Stream/llvm::raw_ostream.
The part worth calling out is the rewrite of the StreamCallback class. Previously this class contained a per-thread buffer of data written. I assume this had something to do with it trying to make sure each log line is delivered as a single event, instead of multiple (possibly interleaved) events. However, this is no longer relevant as the Log class already writes things to a temporary buffer and then delivers the message as a single "write", so I have just removed the code in question.

*** This commit represents a complete reformatting of the LLDB source code *** to conform to clang-format’s LLVM style. This kind of mass change has *** two obvious implications:
Firstly, merging this particular commit into a downstream fork may be a huge effort. Alternatively, it may be worth merging all changes up to this commit, performing the same reformatting operation locally, and then discarding the merge for this particular commit. The commands used to accomplish this reformatting were as follows (with current working directory as the root of the repository):
find . \( -iname "*.c" -or -iname "*.cpp" -or -iname "*.h" -or -iname "*.mm" \) -exec clang-format -i {} + find . -iname "*.py" -exec autopep8 --in-place --aggressive --aggressive {} + ;
The version of clang-format used was 3.9.0, and autopep8 was 1.2.4.
Secondly, “blame” style tools will generally point to this commit instead of a meaningful prior commit. There are alternatives available that will attempt to look through this change and find the appropriate prior commit. YMMV.

Main changes in this patch include: - cleanup plug-in interface and use ConstStrings for plug-in names - Modfiied the BSD Archive plug-in to be able to pick out the correct .o file when .a files contain multiple .o files with the same name by using the timestamp - Modified SymbolFileDWARFDebugMap to properly verify the timestamp on .o files it loads to ensure we don't load updated .o files and cause problems when debugging
The plug-in interface changes:
Modified the lldb_private::PluginInterface class that all plug-ins inherit from:
Changed:
virtual const char * GetPluginName() = 0;
To:
virtual ConstString GetPluginName() = 0;
Removed:
virtual const char * GetShortPluginName() = 0;
- Fixed up all plug-in to adhere to the new interface and to return lldb_private::ConstString values for the plug-in names. - Fixed all plug-ins to return simple names with no prefixes. Some plug-ins had prefixes and most ones didn't, so now they all don't have prefixed names, just simple names like "linux", "gdb-remote", etc.

LLDB is crashing when logging is enabled from lldb-perf-clang. This has to do with the global destructor chain as the process and its threads are being torn down.
All logging channels now make one and only one instance that is kept in a global pointer which is never freed. This guarantees that logging can correctly continue as the process tears itself down.

DWARF with .o files now uses 40-60% less memory!
Big fixes include: - Change line table internal representation to contain "file addresses". Since each line table is owned by a compile unit that is owned by a module, it makes address translation into lldb_private::Address easy to do when needed. - Removed linked address members/methods from lldb_private::Section and lldb_private::Address - lldb_private::LineTable can now relink itself using a FileRangeMap to make it easier to re-link line tables in the future - Added ObjectFile::ClearSymtab() so that we can get rid of the object file symbol tables after we parse them once since they are not needed and kept memory allocated for no reason - Moved the m_sections_ap (std::auto_ptr to section list) and m_symtab_ap (std::auto_ptr to the lldb_private::Symtab) out of each of the ObjectFile subclasses and put it into lldb_private::ObjectFile. - Changed how the debug map is parsed and stored to be able to: - Lazily parse the debug map for each object file - not require the address map for a .o file until debug information is linked for a .o file

Added a mechanism for keeping track of where in the debug information individual Decls came from.
We've had a metadata infrastructure for a while, which was intended to solve a problem we've since dealt with in a different way. (It was meant to keep track of which definition of an Objective-C class was the "true" definition, but we now find it by searching the symbols for the class symbol.) The metadata is attached to the ExternalASTSource, which means it has a one-to-one correspondence with AST contexts.
I've repurposed the metadata infrastructure to hold the object file and DIE offset for the DWARF information corresponding to a Decl. There are methods in ClangASTContext that get and set this metadata, and the ClangASTImporter is capable of tracking down the metadata for Decls that have been copied out of the debug information into the parser's AST context without using any additional memory.
To see the metadata, you just have to enable the expression log: - (lldb) log enable lldb expr - and watch the import messages. The high 32 bits of the metadata indicate the index of the object file in its containing DWARFDebugMap; I have also added a log which you can use to track that mapping: - (lldb) log enable dwarf map -
This adds 64 bits per Decl, which in my testing hasn't turned out to be very much (debugging Clang produces around 6500 Decls in my tests). To track how much data is being consumed, I've also added a global variable g_TotalSizeOfMetadata which tracks the total number of Decls that have metadata in all active AST contexts.
Right now this metadata is enormously useful for tracking down bugs in the debug info parser. In the future I also want to use this information to provide more intelligent error messages instead of printing empty source lines wherever Clang refers to the location where something is defined.

This allows us to see when lookups are being done on functions, addresses, and types by both name and regular expresssion.

Added auto completion for architecture names and for platforms.
Modified the OptionGroupOptions to be able to specify only some of the options that should be appended by using the usage_mask in the group defintions and also provided a way to remap them to a new usage mask after the copy. This allows options to be re-used and also targetted for specific option groups.
Modfied the CommandArgumentType to have a new eArgTypePlatform enumeration. Taught the option parser to be able to automatically use the appropriate auto completion for a given options if nothing is explicitly specified in the option definition. So you don't have to specify it in the option definition tables.
Renamed the default host platform name to "host", and the default platform hostname to be "localhost".
Modified the "file" and "platform select" commands to make sure all options and args are good prior to creating a new platform. Also defer the computation of the architecture in the file command until all options are parsed and the platform has either not been specified or reset to a new value to avoid computing the arch more than once.
Switch the PluginManager code over to using llvm::StringRef for string comparisons and got rid of all the AccessorXXX functions in lieu of the newer mutex + collection singleton accessors.

LLDB now has "Platform" plug-ins. Platform plug-ins are plug-ins that provide an interface to a local or remote debugging platform. By default each host OS that supports LLDB should be registering a "default" platform that will be used unless a new platform is selected. Platforms are responsible for things such as: - getting process information by name or by processs ID - finding platform files. This is useful for remote debugging where there is an SDK with files that might already or need to be cached for debug access. - getting a list of platform supported architectures in the exact order they should be selected. This helps the native x86 platform on MacOSX select the correct x86_64/i386 slice from universal binaries. - Connect to remote platforms for remote debugging - Resolving an executable including finding an executable inside platform specific bundles (macosx uses .app bundles that contain files) and also selecting the appropriate slice of universal files for a given platform.
So by default there is always a local platform, but remote platforms can be connected to. I will soon be adding a new "platform" command that will support the following commands: (lldb) platform connect --name machine1 macosx connect://host:port Connected to "machine1" platform. (lldb) platform disconnect macosx
This allows LLDB to be well setup to do remote debugging and also once connected process listing and finding for things like: (lldb) process attach --name x<TAB>
The currently selected platform plug-in can now auto complete any available processes that start with "x". The responsibilities for the platform plug-in will soon grow and expand.
