Most functions in `net/bluetooth/lib.c` lack propper
documentation.

This patch adds documentation to all exported functions
in `net/bluetoo

Remove __func__ from BT_ERR macro (and the uses).
Prefix "Bluetooth: " to BT_ERR
Remove __func__ from BT_DBG as function can be prefixed when
using dynamic_debug.

3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
