xen/x86: obtain upper 32 bits of video frame buffer address for Dom0
The hypervisor has been supplying this information for a couple of major releases. Make use of it. The need to set a flag in the capabilities field also points out that the prior setting of that field from the hypervisor interface's gbl_caps one was wrong, so that code gets deleted (there's also no equivalent of this in native boot code).

xen: fix wrong SPDX headers of Xen related headers
Commit b24413180f5600 ("License cleanup: add SPDX GPL-2.0 license identifier to files with no license") was meant to do a tree-wide cleanup for files without any license information by adding a SPDX GPL-2.0 line to them.
Unfortunately this was applied even to several Xen-related headers which have been originally under the MIT license, but obviously have been copied to the Linux tree from the Xen project without keeping the license boiler plate as required.
Correct that by changing the license of those files back to "MIT".
Some files still contain the MIT license text. Replace that by the related SPDX line.

xen/privcmd: add IOCTL_PRIVCMD_MMAP_RESOURCE
My recent Xen patch series introduces a new HYPERVISOR_memory_op to support direct priv-mapping of certain guest resources (such as ioreq pages, used by emulators) by a tools domain, rather than having to access such resources via the guest P2M.
This patch adds the necessary infrastructure to the privcmd driver and Xen MMU code to support direct resource mapping.
NOTE: The adjustment in the MMU code is partially cosmetic. Xen will now allow a PV tools domain to map guest pages either by GFN or MFN, thus the term 'mfn' has been swapped for 'pfn' in the lower layers of the remap code.

xen/privcmd: Add IOCTL_PRIVCMD_DM_OP
Recently a new dm_op[1] hypercall was added to Xen to provide a mechanism for restricting device emulators (such as QEMU) to a limited set of hypervisor operations, and being able to audit those operations in the kernel of the domain in which they run.
This patch adds IOCTL_PRIVCMD_DM_OP as gateway for __HYPERVISOR_dm_op.
NOTE: There is no requirement for user-space code to bounce data through locked memory buffers (as with IOCTL_PRIVCMD_HYPERCALL) since privcmd has enough information to lock the original buffers directly.

xen/PMU: Initialization code for Xen PMU
Map shared data structure that will hold CPU registers, VPMU context, V/PCPU IDs of the CPU interrupted by PMU interrupt. Hypervisor fills this information in its handler and passes it to the guest for further processing.
Set up PMU VIRQ.
Now that perf infrastructure will assume that PMU is available on a PV guest we need to be careful and make sure that accesses via RDPMC instruction don't cause fatal traps by the hypervisor. Provide a nop RDPMC handler.
For the same reason avoid issuing a warning on a write to APIC's LVTPC.
Both of these will be made functional in later patches.

arm: xen: implement multicall hypercall support.
As part of this make the usual change to xen_ulong_t in place of unsigned long. This change has no impact on x86.
The Linux definition of struct multicall_entry.result differs from the Xen definition, I think for good reasons, and used a long rather than an unsigned long. Therefore introduce a xen_long_t, which is a long on x86 architectures and a signed 64-bit integer on ARM.
Use uint32_t nr_calls on x86 for consistency with the ARM definition.
Build tested on amd64 and i386 builds. Runtime tested on ARM.

Add the hypercall sub-ops and the structures for the shared data used in the FIFO-based event channel ABI.
The design document for this new ABI is available here:
http://xenbits.xen.org/people/dvrabel/event-channels-H.pdf
In summary, events are reported using a per-domain shared event array of event words. Each event word has PENDING, LINKED and MASKED bits and a LINK field for pointing to the next event in the event queue.
There are 16 event queues (with different priorities) per-VCPU.
Key advantages of this new ABI include:
- Support for over 100,000 events (2^17). - 16 different event priorities. - Improved fairness in event latency through the use of FIFOs.
The ABI is available in Xen 4.4 and later.

xen: Introduce xen_pfn_t for pfn and mfn types
All the original Xen headers have xen_pfn_t as mfn and pfn type, however when they have been imported in Linux, xen_pfn_t has been replaced with unsigned long. That might work for x86 and ia64 but it does not for arm. Bring back xen_pfn_t and let each architecture define xen_pfn_t as they see fit.

xen/granttable: Introducing grant table V2 stucture
This patch introduces new structures of grant table V2, grant table V2 is an extension from V1. Grant table is shared between guest and Xen, and Xen is responsible to do corresponding work for grant operations, such as: figure out guest's grant table version, perform different actions based on different grant table version, etc. Although full-page structure of V2 is different from V1, it play the same role as V1.

xen: cleancache shim to Xen Transcendent Memory
This patch provides a shim between the kernel-internal cleancache API (see Documentation/mm/cleancache.txt) and the Xen Transcendent Memory ABI (see http://oss.oracle.com/projects/tmem).
Xen tmem provides "hypervisor RAM" as an ephemeral page-oriented pseudo-RAM store for cleancache pages, shared cleancache pages, and frontswap pages. Tmem provides enterprise-quality concurrency, full save/restore and live migration support, compression and deduplication.
A presentation showing up to 8% faster performance and up to 52% reduction in sectors read on a kernel compile workload, despite aggressive in-kernel page reclamation ("self-ballooning") can be found at:

xen: implement ptep_modify_prot_start/commit
Xen has a pte update function which will update a pte while preserving its accessed and dirty bits. This means that ptep_modify_prot_start() can be implemented as a simple read of the pte value. The hardware may update the pte in the meantime, but ptep_modify_prot_commit() updates it while preserving any changes that may have happened in the meantime.
The updates in ptep_modify_prot_commit() are batched if we're currently in lazy mmu mode.
The mmu_update hypercall can take a batch of updates to perform, but this code doesn't make particular use of that feature, in favour of using generic multicall batching to get them all into the hypervisor.
The net effect of this is that each mprotect pte update turns from two expensive trap-and-emulate faults into they hypervisor into a single hypercall whose cost is amortized in a batched multicall.
