kfifo: don't include dma-mapping.h in kfifo.h

Nothing in kfifo.h directly needs dma-mapping.h, only two macros
use DMA_MAPPING_ERROR when actually instantiated. Drop the

use DMA_MAPPING_ERROR when actually instantiated. Drop the
dma-mapping.h include to reduce include bloat.

Add an explicity <linux/io.h> include to drivers/mailbox/omap-mailbox.c
as that file uses __raw_readl and __raw_writel through a complicated
include chain involving <linux/dma-mapping.h>

need a wrapper around kfifo which provides us with a tail (sometimes
"tail" offset, sometimes a pointer) to the kfifo data. And which returns
count of available data -- but not larger than to the end of the buffer
(hence _linear in the names). I.e. something like CIRC_CNT_TO_END() in
the legacy circ_buf.

This patch adds such two helpers.

kfifo_skip_count() is an extended version of kfifo_skip(), accepting
also count. This will be useful in the serial code later.

Now, it can be used to implement both kfifo_skip() and
kfifo_dma_out_finish(). In the latter, 'len' only needs to be divided by
'type' size (as it was until now).

And stop using statement expressions when the return value is cast to
'void'. Use classic 'do {} while (0)' instead.

Note: perhaps we should skip 'count' records for the 'recsize' case, but
the original (kfifo_dma_out_finish()) used to skip only one record. So
this is kept unchanged and 'count' is still ignored in the recsize case.

The kfifo_to_user() macro is supposed to return zero for success or
negative error codes. Unfortunately, there is a signedness bug so it
returns unsigned int.

returns unsigned int. This only affects callers which try to save the
result in ssize_t and as far as I can see the only place which does that
is line6_hwdep_read().

TL;DR: s/_uint/_int/.

If you try to store u64 in a kfifo (or a struct with u64 members),
then the buf member of __STRUCT_KFIFO_PTR will cause 4 bytes
padding

If you try to store u64 in a kfifo (or a struct with u64 members),
then the buf member of __STRUCT_KFIFO_PTR will cause 4 bytes
padding due to alignment (note that struct __kfifo is 20 bytes
on 32 bit).

That in turn causes the __is_kfifo_ptr() to fail, which is caught
by kfifo_alloc(), which now returns EINVAL.

So, ensure that __is_kfifo_ptr() compares to the right structure.

This patch enhances the type safety for the kfifo API. It is now safe
to put const data into a non const FIFO and the API will now generate a
compiler warning when reading from the fifo where the destination
address is pointing to a const variable.

As a side effect the kfifo_put() does now expect the value of an element
instead a pointer to the element. This was suggested Russell King. It
make the handling of the kfifo_put easier since there is no need to
create a helper variable for getting the address of a pointer or to pass
integers of different sizes.

IMHO the API break is okay, since there are currently only six users of
kfifo_put().

The code is also cleaner by kicking out the "if (0)" expressions.

will fail to detect the error.
