s390 allocates+prepares the elfcore hdr in the dump (2nd) kernel, not in the crashed kernel.
RAM provided by memory devices such as virtio-mem can only be detected using the device driver; when vmcore_init() is called, these device drivers are usually not loaded yet, or the devices did not get probed yet. Consequently, on s390 these RAM ranges will not be included in the crash dump, which makes the dump partially corrupt and is unfortunate.
Instead of deferring the vmcore_init() call, to an (unclear?) later point, let's reuse the vmcore_cb infrastructure to obtain device RAM ranges as the device drivers probe the device and get access to this information.
Then, we'll add these ranges to the vmcore, adding more PT_LOAD entries and updating the offsets+vmcore size.
Use a separate Kconfig option to be set by an architecture to include this code only if the arch really needs it. Further, we'll make the config depend on the relevant drivers (i.e., virtio_mem) once they implement support (next). The alternative of having a PROVIDE_PROC_VMCORE_DEVICE_RAM config option was dropped for now for simplicity.
The current target use case is s390, which only creates an elf64 elfcore, so focusing on elf64 is sufficient.

We'll move "struct vmcoredd_node" to vmcore.c, because it is only used internally. While "struct vmcore" is only used internally for now, we're planning on using it from inline functions in crash_dump.h next, so move it to crash_dump.h.
While at it, rename "struct vmcore" to "struct vmcore_range", which is a more suitable name and will make the usage of it outside of vmcore.c clearer.

Currently, is_kdump_kernel() returns true when elfcorehdr_addr is set. While elfcorehdr_addr is set for kexec based kernel dump mechanism, alternate dump capturing methods like fadump [1] also set it to export the vmcore. Since, is_kdump_kernel() is used to restrict resources in crash dump capture kernel and such restrictions may not be desirable for fadump, allow is_kdump_kernel() to be defined differently for such scenarios. With this, is_kdump_kernel() could be false while vmcore is usable. So, remove unnecessary dependency with is_kdump_kernel(), for exporting vmcore.

For some reason several people have been sending bad patches to fix compiler warnings in vmcore recently. Here's how it should be done. Compile-tested only on x86. As noted in the first patch, s390 should take this conversion a bit further, but I'm not inclined to do that work myself.
This patch (of 3):
Instead of passing in a 'buf' and 'userbuf' argument, pass in an iov_iter. s390 needs more work to pass the iov_iter down further, or refactor, but I'd be more comfortable if someone who can test on s390 did that work.
It's more convenient to convert the whole of read_from_oldmem() to take an iov_iter at the same time, so rename it to read_from_oldmem_iter() and add a temporary read_from_oldmem() wrapper that creates an iov_iter.

Let's support multiple registered callbacks, making sure that registering vmcore callbacks cannot fail. Make the callback return a bool instead of an int, handling how to deal with errors internally. Drop unused HAVE_OLDMEM_PFN_IS_RAM.
We soon want to make use of this infrastructure from other drivers: virtio-mem, registering one callback for each virtio-mem device, to prevent reading unplugged virtio-mem memory.
Handle it via a generic vmcore_cb structure, prepared for future extensions: for example, once we support virtio-mem on s390x where the vmcore is completely constructed in the second kernel, we want to detect and add plugged virtio-mem memory ranges to the vmcore in order for them to get dumped properly.
Handle corner cases that are unexpected and shouldn't happen in sane setups: registering a callback after the vmcore has already been opened (warn only) and unregistering a callback after the vmcore has already been opened (warn and essentially read only zeroes from that point on).

Make the forward declarations of elfcorehdr_addr and elfcorehdr_size, and the definitions of ELFCORE_ADDR_MAX and ELFCORE_ADDR_ERR always available, like is done for phys_initrd_start and phys_initrd_size. Code referring to these symbols can then just check for IS_ENABLED(CONFIG_CRASH_DUMP), instead of requiring conditional compilation using an #ifdef, thus preparing to increase compile coverage.

Secure Encrypted Virtualization is an x86-specific feature, so it shouldn't appear in generic kernel code because it forces non-x86 architectures to define the sev_active() function, which doesn't make a lot of sense.
To solve this problem, add an x86 elfcorehdr_read() function to override the generic weak implementation. To do that, it's necessary to make read_from_oldmem() public so that it can be used outside of vmcore.c.
Also, remove the export for sev_active() since it's only used in files that won't be built as modules.

The sequence of actions done by device drivers to append their device specific hardware/firmware logs to /proc/vmcore are as follows:
1. During probe (before hardware is initialized), device drivers register to the vmcore module (via vmcore_add_device_dump()), with callback function, along with buffer size and log name needed for firmware/hardware log collection.
2. vmcore module allocates the buffer with requested size. It adds an Elf note and invokes the device driver's registered callback function.
3. Device driver collects all hardware/firmware logs into the buffer and returns control back to vmcore module.
Ensure that the device dump buffer size is always aligned to page size so that it can be mmaped.
Also, rename alloc_elfnotes_buf() to vmcore_alloc_buf() to make it more generic and reserve NT_VMCOREDD note type to indicate vmcore device dump.

License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0' SPDX license identifier. The SPDX identifier is a legally binding shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of the use cases: - file had no licensing information it it. - file was a */uapi/* one with no licensing information in it, - file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases where non-standard license headers were used, and references to license had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to a file was done in a spreadsheet of side by side results from of the output of two independent scanners (ScanCode & Windriver) producing SPDX tag:value files created by Philippe Ombredanne. Philippe prepared the base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files assessed. Kate Stewart did a file by file comparison of the scanner results in the spreadsheet to determine which SPDX license identifier(s) to be applied to the file. She confirmed any determination that was not immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was: - Files considered eligible had to be source code files. - Make and config files were included as candidates if they contained >5 lines of source - File already had some variant of a license header in it (even if <5 lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license identifiers to apply.
- when both scanners couldn't find any license traces, file was considered to have no license information in it, and the top level COPYING file license applied.
For non */uapi/* files that summary was:

and resulted in the first patch in this series.
If that file was a */uapi/* path one, it was "GPL-2.0 WITH Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was:

and resulted in the second patch in this series.
- if a file had some form of licensing information in it, and was one of the */uapi/* ones, it was denoted with the Linux-syscall-note if any GPL family license was found in the file or had no licensing in it (per prior point). Results summary:

and that resulted in the third patch in this series.
- when the two scanners agreed on the detected license(s), that became the concluded license(s).
- when there was disagreement between the two scanners (one detected a license but the other didn't, or they both detected different licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file resulted in a clear resolution of the license that should apply (and which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier, the file was flagged for further research and to be revisited later in time.
In total, over 70 hours of logged manual review was done on the spreadsheet to determine the SPDX license identifiers to apply to the source files by Kate, Philippe, Thomas and, in some cases, confirmation by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from FOSSology, and compared selected files where the other two scanners disagreed against that SPDX file, to see if there was new insights. The Windriver scanner is based on an older version of FOSSology in part, so they are related.
Thomas did random spot checks in about 500 files from the spreadsheets for the uapi headers and agreed with SPDX license identifier in the files he inspected. For the non-uapi files Thomas did random spot checks in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have copy/paste license identifier errors, and have been fixed to reflect the correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual inspection and review of the 12,461 patched files from the initial patch version early this week with: - a full scancode scan run, collecting the matched texts, detected license ids and scores - reviewing anything where there was a license detected (about 500+ files) to ensure that the applied SPDX license was correct - reviewing anything where there was no detection but the patch license was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This worksheet was then exported into 3 different .csv files for the different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to parse the csv files and add the proper SPDX tag to the file, in the format that the file expected. This script was further refined by Greg based on the output to detect more types of files automatically and to distinguish between header and source .c files (which need different comment types.) Finally Greg ran the script using the .csv files to generate the patches.

fs/proc/vmcore.c provides default definitions explicitly marked "weak". arch/s390 provides its own definitions intended to override the default ones, but the "weak" attribute on the declarations applied to the s390 definitions as well, so the linker chose one based on link order (see 10629d711ed7 ("PCI: Remove __weak annotation from pcibios_get_phb_of_node decl")).
Remove the "weak" attribute from the declarations so we always prefer a non-weak definition over the weak one, independent of link order.

For zfcpdump we can't map the HSA storage because it is only available via a read interface. Therefore, for the new vmcore mmap feature we have introduce a new mechanism to create mappings on demand.
This patch introduces a new architecture function remap_oldmem_pfn_range() that should be used to create mappings with remap_pfn_range() for oldmem areas that can be directly mapped. For zfcpdump this is everything besides of the HSA memory. For the areas that are not mapped by remap_oldmem_pfn_range() a generic vmcore a new generic vmcore fault handler mmap_vmcore_fault() is called.
This handler works as follows:
* Get already available or new page from page cache (find_or_create_page) * Check if /proc/vmcore page is filled with data (PageUptodate) * If yes: Return that page * If no: Fill page using __vmcore_read(), set PageUptodate, and return page

vmcore: introduce ELF header in new memory feature
For s390 we want to use /proc/vmcore for our SCSI stand-alone dump (zfcpdump). We have support where the first HSA_SIZE bytes are saved into a hypervisor owned memory area (HSA) before the kdump kernel is booted. When the kdump kernel starts, it is restricted to use only HSA_SIZE bytes.
The advantages of this mechanism are:
* No crashkernel memory has to be defined in the old kernel. * Early boot problems (before kexec_load has been done) can be dumped * Non-Linux systems can be dumped.
We modify the s390 copy_oldmem_page() function to read from the HSA memory if memory below HSA_SIZE bytes is requested.
Since we cannot use the kexec tool to load the kernel in this scenario, we have to build the ELF header in the 2nd (kdump/new) kernel.
So with the following patch set we would like to introduce the new function that the ELF header for /proc/vmcore can be created in the 2nd kernel memory.
The following steps are done during zfcpdump execution:
1. Production system crashes 2. User boots a SCSI disk that has been prepared with the zfcpdump tool 3. Hypervisor saves CPU state of boot CPU and HSA_SIZE bytes of memory into HSA 4. Boot loader loads kernel into low memory area 5. Kernel boots and uses only HSA_SIZE bytes of memory 6. Kernel saves registers of non-boot CPUs 7. Kernel does memory detection for dump memory map 8. Kernel creates ELF header for /proc/vmcore 9. /proc/vmcore uses this header for initialization 10. The zfcpdump user space reads /proc/vmcore to write dump to SCSI disk - copy_oldmem_page() copies from HSA for memory below HSA_SIZE - copy_oldmem_page() copies from real memory for memory above HSA_SIZE
Currently for s390 we create the ELF core header in the 2nd kernel with a small trick. We relocate the addresses in the ELF header in a way that for the /proc/vmcore code it seems to be in the 1st kernel (old) memory and the read_from_oldmem() returns the correct data. This allows the /proc/vmcore code to use the ELF header in the 2nd kernel.
This patch:
Exchange the old mechanism with the new and much cleaner function call override feature that now offcially allows to create the ELF core header in the 2nd kernel.
To use the new feature the following function have to be defined by the architecture backend code to read from new memory:
* elfcorehdr_alloc: Allocate ELF header * elfcorehdr_free: Free the memory of the ELF header * elfcorehdr_read: Read from ELF header * elfcorehdr_read_notes: Read from ELF notes
