ata: Switch back to struct platform_driver::remove()
After commit 0edb555a65d1 ("platform: Make platform_driver::remove() return void") .remove() is (again) the right callback to implement for platform drivers.
Convert all platform drivers below drivers/ata/ to use .remove(), with the eventual goal to drop struct platform_driver::remove_new(). As .remove() and .remove_new() have the same prototypes, conversion is done by just changing the structure member name in the driver initializer.
While touching these files, make indention of the struct initializer consistent in several files.

ata: libata: Make ata_platform_remove_one return void
The function returned zero unconditionally, so the function returning an int is something between useless and irritating. With the goal to make platform drivers' remove function return void, it's helpful to convert the function accordingly. This converts several drivers to the new .remove_new callback that was introduced to smoothen the platform driver conversion.

ata: pata_platform: Fix a NULL pointer dereference in __pata_platform_probe()
In __pata_platform_probe(), devm_kzalloc() is assigned to ap->ops and there is a dereference of it right after that, which could introduce a NULL pointer dereference bug.
Fix this by adding a NULL check of ap->ops.
This bug was found by a static analyzer.
Builds with 'make allyesconfig' show no new warnings, and our static analyzer no longer warns about this code.

ata: pata_of_platform: Allow to use 16-bit wide data transfer
In some cases, the system bus can be configured for 16-bit mode, in this case using read/write functions for 32-bit values results in two cycles of 16 bits each, which is wrong. This patch adds the devicetree flag to switch the driver to use 16-bit mode for I/O transfers.

libata: remove ata_sff_data_xfer_noirq()
ata_sff_data_xfer_noirq() is invoked via the ->sff_data_xfer hook. The latter is invoked by ata_pio_sector(), atapi_send_cdb() and __atapi_pio_bytes() which in turn is invoked by ata_sff_hsm_move(). The latter function requires that the "ap->lock" lock is held which needs to be taken with disabled interrupts.
There is no need have to have ata_sff_data_xfer_noirq() which invokes ata_sff_data_xfer32() with disabled interrupts because at this point the interrupts are already disabled. Remove the function and its references to it and replace all callers with ata_sff_data_xfer32().

ata: pata_platform: fix owner module reference mismatch for scsi host
The owner module reference of the pata_of_platform's scsi_host is initialized to pata_platform's one, because pata_of_platform driver use a scsi_host_template defined in pata_platform. So this drivers can be unloaded even if the scsi device is being accessed.
This fixes it by propagating the scsi_host_template to pata_of_platform driver. The scsi_host_template is passed through a new argument of __pata_platform_probe().

CONFIG_HOTPLUG is going away as an option. As a result, the __dev* markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata, and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me in order to handle some of the coding style issues better, by hand.

libata-sff: clean up BMDMA initialization
When BMDMA initialization failed or BMDMA was not available for whatever reason, bmdma_addr was left at zero and used as an indication that BMDMA shouldn't be used. This leads to the following problems.
p1. For BMDMA drivers which don't use traditional BMDMA register, ata_bmdma_mode_filter() incorrectly inhibits DMA modes. Those drivers either have to inherit from ata_sff_port_ops or clear ->mode_filter explicitly.
p2. non-BMDMA drivers call into BMDMA PRD table allocation. It doesn't actually allocate PRD table if bmdma_addr is not initialized but is still confusing.
p3. For BMDMA drivers which don't use traditional BMDMA register, some methods might not be invoked as expected (e.g. bmdma_stop from ata_sff_post_internal_cmd()).
p4. SFF drivers w/ custom DMA interface implement noop BMDMA ops worrying libata core might call into one of them.
These problems are caused by the muddy line between SFF and BMDMA and the assumption that all BMDMA controllers initialize bmdma_addr.
This patch fixes p1 and p2 by removing the bmdma_addr assumption and moving prd allocation to BMDMA port start. Later patches will fix the remaining issues.
This patch improves BMDMA initialization such that
* When BMDMA register initialization fails, falls back to PIO instead of failing. ata_pci_bmdma_init() never fails now.
* When ata_pci_bmdma_init() falls back to PIO, it clears ap->mwdma_mask and udma_mask instead of depending on ata_bmdma_mode_filter(). This makes ata_bmdma_mode_filter() unnecessary thus resolving p1.
* ata_port_start() which actually is BMDMA specific is moved to ata_bmdma_port_start(). ata_port_start() and ata_sff_port_start() are killed.
* ata_sff_port_start32() is moved and renamed to ata_bmdma_port_start32().
Drivers which no longer call into PRD table allocation are...
pdc_adma, sata_inic162x, sata_qstor, sata_sx4, pata_cmd640 and all drivers which inherit from ata_sff_port_ops.
pata_icside sets ->port_start to ATA_OP_NULL as it doesn't need PRD but is a BMDMA controller and doesn't have custom port_start like other such controllers.
Note that with the previous patch which makes all and only BMDMA drivers inherit from ata_bmdma_port_ops, this change doesn't break drivers which need PRD table.

libata: beef up iterators
There currently are the following looping constructs.
* __ata_port_for_each_link() for all available links * ata_port_for_each_link() for edge links * ata_link_for_each_dev() for all devices * ata_link_for_each_dev_reverse() for all devices in reverse order
Now there's a need for looping construct which is similar to __ata_port_for_each_link() but iterates over PMP links before the host link. Instead of adding another one with long name, do the following cleanup.
* Implement and export ata_link_next() and ata_dev_next() which take @mode parameter and can be used to build custom loop. * Implement ata_for_each_link() and ata_for_each_dev() which take looping mode explicitly.
The following iteration modes are implemented.
* ATA_LITER_EDGE : loop over edge links * ATA_LITER_HOST_FIRST : loop over all links, host link first * ATA_LITER_PMP_FIRST : loop over all links, PMP links first
* ATA_DITER_ENABLED : loop over enabled devices * ATA_DITER_ENABLED_REVERSE : loop over enabled devices in reverse order * ATA_DITER_ALL : loop over all devices * ATA_DITER_ALL_REVERSE : loop over all devices in reverse order
This change removes exlicit device enabledness checks from many loops and makes it clear which ones are iterated over in which direction.
