qsort.3: Bump Dd and note that Annex K is optional

Add qsort_s(3). Apart from the constraints, it also makes it easier
to port software written for Linux variant of qsort_r(3).

Renumber clauses to reduce diffs to other versions

NetBSD, OpenBSD, and Android's Bionic number the clauses 1 through 3,
so follow suit to make comparison easier.

- Do not cast away const.
- Use the canonical idiom to compare two numbers.
- Use proper type for sizes, i.e. size_t ins
